Client Application

To begin ordering hair extensions and supplies from Diamond Hair Extensions, we require your details to be provided on our Client Application Form.

This form should be printed out and either handed to our representitve, or faxed to the number on the form.

All purchases from your salon must be paid for by credit card, details of which must be included on the appication form.

Customer Number
For security purposes, to avoid unauthorised orders, you will be provided with a unique customer number

This customer number will be checked against each order received from your salon, prior to charging and dispatch. You should keep this number safe, and only provide it to persons in your employ who are authorised by you to place orders on your behalf.

Should you wish to change this number at any future time, please advise us to forward a new customer number to you via email.

Privacy
Your privacy is always respected. We will never intentionally disclose or pass on individually identifiable information about our customers to any third party.
